CIVIL REGISTRY

Civil registration is the continuous, permanent, and compulsory recording of vital events occurring in the life of an individual such as birth, marriage, and death, as well as court decrees, and legal instruments affecting civil status in appropriate registers in the Philippines as mandated by Act No. 3753, the Civil Registry Law.

The Report of Birth presents many facts about an individual such as the person's name, date and place of birth, parents, religion, and citizenship, among others; the Report of Marriage establishes a change in civil status and the legal spouse; and the Report of Death records the passing away of an individual.

All reports of birth, marriage and death filed with the Consulate are forwarded to the Philippine Statistics Authority (PSA), formerly called the National Statistics Office (NSO), in the Philippines through the Department of Foreign Affairs.

These records are important in legal and personal transactions, applying for jobs, obtaining passports for travel, entrance to school, claiming insurance benefits, and others; and statistically, these records provide data on the number of births, deaths, marriages, fetal deaths, adoptions and the like, which in turn are essential in development planning.

PSA / NSO copies of all submitted reports may be ordered online at psaserbilis.com.ph or in person at the PSA in the Philippines six (6) months after the filing of the Reports at the Consulate.

The Philippine Consulate General in Toronto processes the civil registration of vital events that occurr/ed in Ontario (except Eastern Ontario and Ottawa-Gatineau Capital Region) and Manitoba only. REPORT OF BIRTH

For recording of births abroad of children born to Filipino parent(s), the following are required:
  1. Four (4) original duly accomplished Report of Birth forms (TYPEWRITTEN AND PRINTED; and when submitting by mail, duly notarized)
  2. One (1) original and four (4) photocopies of Certified Copy of Birth Registration issued by the Office of the Registrar General (for Ontario) and Vital Statistics Agency (for Manitoba)
  3. One (1) original and four (4) photocopies of the parents’ marriage certificate (i.e., Marriage Certificate issued by the Philippine Statistics Authority / approved Report of Marriage)
  4. One (1) Original and four (4) photocopies each of the data page of the passports of the parents
  5. One (1) Original and four (4) photocopies of immigration status record of parents at the time of child’s birth (i.e., Canadian Citizenship Certificate/Card, PR Card, Work/Student Permit, or Visitor's Visa; and Philippine Dual Citizenship Identification Certificate, if applicable)
  6. If the child was included in a parent’s approved petition for retention/re-acquisition of Philippine citizenship, four (4) photocopies of the child’s Dual Citizenship Identification Certificate or Order of Approval/Petition for Reacquisition of Philippine Citizenship, indicating the name of the child as a minor-beneficiary of the parent’s reacquisition of Philippine citizenship
  7. If reporting one (1) year after the date of birth, one (1) original and three (3) photocopies of Affidavit of Delayed Registration of Birth (when submitting by mail, duly notarized)
  8. 8. If the child is 18 years of age or older, one (1) original and four (4) photocopies of the "Negative Certification of Birth" issued by the Philippine Statistics Authority (Please refer to www.psaserbilis.com.ph for ordering online.)
  9. Conditional requirements:
    1. If parents were not married at the time of child’s birth and mother had no prior subsisting marriage:
      1. Affidavit of Admission of Paternity(AAP) executed by the Father, if the father is willing to acknowledge paternity
      2. Affidavit to Use the Surname of the Father (AUSF)
        1. Executed by the Mother, if the child is below 7 years old
        2. Executed by the child and attested by the mother, if the child is between 7 and 17 years old
        3. Executed by the child, if the child is 18 years old and above
        Both affidavits have to be executed in four (4) originals either in person at the Consulate or before a notary public and duly Apostillized by the appropriate competent authority if sent by mail. The Consulate will issue a Certificate of Registration for the AAP and/or AUSF.
    2. If child’s parents were subsequently married, four (4) originals of the Joint Affidavit of Legitimation (when submitting by mail, duly notarized)
  10. If submitting in person, book an appointment here. If submitting by mail, provide a self-addressed stamped mailing envelope, preferably XPRESSPOST from CANADA POST for tracking purposes. Include your telephone number and/or email address in case we need to contact you.

    The Consular Officer reserves the right to require additional proof or documents from an applicant, to prove his/her citizenship or identity, and ensure accurate and complete personal data entries.

    Processing Report of Birth applications takes around five (5) business days upon receipt of the submission with complete supporting documents. This processing time excludes delivery time for release by mail.

  11. Processing Fees (non-refundable):
    • CAD$36.25 for Report of Birth
    • CAD$36.25 for Affidavit of Delayed Registration of Birth
    • CAD$36.25 for AAP
    • CAD$36.25 for AUSF
    • CAD$36.25 for Certificate of Registration of Legal Instrument for those executing AAP and/or AUSF
    • CAD$36.25 for Joint Affidavit of Legitimation

REPORT OF MARRIAGE

For reporting of marriage OF FILIPINO CITIZENS solemnized in Canada, the following are required:
  1. Four (4) original duly accomplished lReport of Marriage (ROM) forms (TYPEWRITTEN AND PRINTED; to be signed by both parties in person at the Consulate or duly notarized when submitting by mail)
  2. One (1) original and four (4) photocopies of the Certified Copy of Marriage Registration (long form) issued by the Office of the Civil Registrar General (for Ontario) or Registration of Marriage issued by Vital Statistics (for Manitoba)
  3. One (1) original and four (4) photocopies of the birth certificates of the contracting parties (MUST BE issued by the Philippine Statistics Authority for Filipinos/former Filipinos)
  4. One (1) original and four (4) photocopies of the data page of the passports of the contracting parties
  5. Original and four (4) photocopies of immigration status record of contracting parties at the time of marriage (i.e., Canadian Citizenship Certificate/Card, PR Card, Work/Student Permit, or Visitor's Visa; and Philippine Dual Citizenship Identification Certificate, if applicable)
  6. If reporting one (1) year after the date of marriage, one (1) original and three (3) photocopies of Affidavit of Delayed Registration of Marriage (when submitting by mail, must be duly notarized)
  7. If marriage was solemnized more than ten (10) years ago, one (1) original and three (3) photocopies of the “Certificate of No Marriage Record” (CENOMAR) issued by the Philippine Statistics Authority (Please refer to www.psaserbilis.com.ph for ordering online)
  8. Conditional requirements:
    1. Where one or both of the contracting parties had a previous marriage annulled, one (1) original and four (4) photocopies of the PSA Marriage Certificate with Annotation stating that the marriage has been declared null and void (for Filipino spouse) and/or one (1) original and four (4) photocopies of the Court Order declaring the marriage annulled (for foreign spouse)
    2. Where one or both of the contracting parties is divorced, one (1) original and four (4) photocopies of the PSA Marriage Certificate with Annotation on the recognition of the foreign judgment of divorce (for Filipino spouse) and/or one (1) original and four (4) photocopies of the Final Decree of Divorce (for foreign spouse)
    3. c. Where one or both of the contracting parties is widowed, one (1) original and four (4) photocopies of the Death Certificate (MUST BE issued by the Philippine Statistics Authority if the deceased was Filipino)
  9. If submitting in person, book an appointment here. If submitting by mail, provide a self-addressed stamped mailing envelope, preferably XPRESSPOST from CANADA POST for tracking purposes. Include your telephone number and/or email address in case we need to contact you.

REPORT OF DEATH

For reporting the death of a Filipino national, the following are required:
  1. Four (4) original duly accomplished Report of Death forms (TYPEWRITTEN AND PRINTED; to be signed by the informant in person at the Consulate or duly notarized when submitting by mail)
  2. One (1) original and four (4) photocopies of the STATEMENT OF DEATH (Form 15) issued by the Office of the Registrar General
  3. One (1) original and four (4) photocopies of the MEDICAL CERTIFICATE OF DEATH (Form 16) issued by the Office of the Registrar General
  4. One (1) original and four (4) photocopies of the data page of the Philippine passport of the deceased
  5. Original and four (4) photocopies of the decedent’s Canadian immigration status document (i.e., Canadian Citizenship Certificate/Card, PR Card, Work/Student Permit, or Visitor's Visa) or Philippine Dual Citizenship Identification Certificate, if applicable
  6. If submitting in person, book an appointment here. If submitting by mail, provide a self-addressed stamped mailing envelope, preferably an XPRESSPOST from CANADA POST for tracking purposes. Include your telephone number and/or email address in case we need to contact you

MORTUARY CERTIFICATE

(For Shipment of Remains)

LIST OF REQUIREMENTS

The following documents must be submitted

  1. Four (4) photocopies of the deceased’s passport data page;
  2. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) (if death occurred in Ontario), or REGISTRATION OF DEATH (if death occurred in Manitoba) issued by the office of Registrar General;
  3. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) (Ontario and Manitoba) issued by the Office of Registrar General;
  4. One (1) original and three (3) photocopies of EMBALMER’S CERTIFICATE with the declaration that the casket contains only the remains of the deceased, among others (under oath before a Notary Public or person authorized to administer);
  5. One (1) original and three (3) photocopies of MEDICAL HEALTH OFFICER CERTIFICATE;
  6. One (1) original and three (3) photocopies of Certificate for SHIPMENT OF BODY OUTSIDE OF ONTARIO (if death occurred in Ontario) or Certificate for SHIPMENT OF BODY OUT OF PROVINCE (if death occurred in Manitoba);
  7. One (1) original and three (3) photocopies of BEREAVEMENT AUTHORITY OF ONTARIO CERTIFICATE (if death occurred in Ontario), or FUNERAL BOARD OF MANITOBA CERTIFICATE/LETTER (if death occurred in Manitoba); and
  8. Four (4) copies of FLIGHT ITINERARY of the shipment containing the flight details, information of the consignee (name of consignee, complete address, and telephone number) and AIRWAY BILL number.
  • Fee of CAD$36.25 for five (5) working days processing.
  • For expedited processing: Additional fee of CAD$14.50
  • Applicants can pay in cash or money order/bank draft payable to the Philippine Consulate General in Toronto (personal cheques are not accepted). Alternatively, applicants can opt to pay electronically using credit card, debit card, Apple Pay or Google Pay.

Note:

If the deceased is a Filipino national at the time of death, a Report of Death should be filed with the following additional requirements:

  1. Duly accomplished Report of Death Forms . (in four originals, TO BE ACCOMPLISHED IN BLACK INK) signed by the informant for the deceased. (ROD forms should be duly notarized when reporting by mail)
  2. Four (4) photocopies of any proof of the Filipino Citizenship of the deceased (e.g. Passport).
  3. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) issued by the Office of the Registrar General.
  4. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) issued by the Office of the Registrar General.

Fee for Report of Death:

CAD$36.25

Expedited Processing: Additional Fee of CAD$14.50

MORTUARY CERTIFICATE

(For Shipment of Cremated Remains/Ashes)

LIST OF REQUIREMENTS

For SHIPMENT OF CREMATED REMAINS/ASHES properly contained in an urn, the following documents must be submitted:

  1. Four (4) photocopies of the deceased’s passport data page;
  2. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) (if death occurred in Ontario), or REGISTRATION OF DEATH (if death occurred in Manitoba) issued by the office of Registrar General;
  3. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) (Ontario and Manitoba) issued by the Office of Registrar General;
  4. One (1) original and three (3) photocopies of CERTIFICATE OF CREMATION stating that the urn contains only the ashes of the deceased;
  5. Four (4) photocopies of the passport data page of the person bringing the urn;
  6. Four (4) photocopies of FLIGHT ITINERARY of the person bringing the urn;

Note: Applicant need not bring the urn to the Philippine Consulate for the purpose of applying a Mortuary Certificate.

Fee of CAD$36.25 for five (5) working days processing.

Expedited Processing: Additional Fee of CAD$14.50

  2. Four (4) photocopies of any proof of the Filipino Citizenship of the deceased (e.g. Passport)
  3. One (1) original and three (3) photocopies of STATEMENT OF DEATH (Form 15) issued by the Office of the Registrar General.
  4. One (1) original and three (3) photocopies of MEDICAL CERTIFICATE OF DEATH (Form 16) issued by the Office of the Registrar General.

Report of Death forms should be duly notarized when reporting by mail.
